Shear Intolerance
Shear intolerance is related to spinal instability and a lack of strength in the posterior lumbar musculature.
Bending forward allows gravity to cause micromovements in the lumbar vertebrae. Normally, these shear forces are opposed by the posterior lumbar muscles, but injury and disuse, as well as inappropriate loading, mean these muscles may not be strong enough, and hence, activities which involve shear forces become painful. Even simple tasks like opening a window can cause pain from shear intolerance.
Shear intolerance can be the primary cause of low back pain, but typically involves other factors, generally flexion-related back pain.

Pain from shear intolerance can occur from gym-based activities such as a kettlebell swing, but can also happen in daily activities such as pushing a shopping trolley, opening doors, opening and closing windows, reaching overhead and even flushing a toilet or brushing teeth.
It may develop as a consequence of another injury, disuse, or simply inappropriate loading for the current tissue capacity.
Core stability training and developing strength in the posterior lumbar muscles is the mainstay of rehabilitation with this issue.
How we work
A thorough history and assessment will reveal mechanisms of injury, which can include poor movement patterns, dysfunctional motor patterns, muscle weakness, mobility restrictions, perturbed postures and inappropriate loads.
Rehabilitation focuses on first removing the pain-causing motions, postures and loads, then developing appropriate core stability, teaching better movement patterns, re-establishing correct motor patterns, addressing mobility restrictions, then building focused strength and restoring proprioception.
