Why does your back hurt when carrying things? It’s most likely load intolerance . Load intolerance simply means that the load being out through the body, is causing compresssive forces through the spine to trigger pain. “Load” can mean any action, such as pushing a wheelbarrow or shopping trolley, opening doors, gardening or using a lawnmower, as well as more obvious activities like carrying shopping and laundry baskets, moving furniture, playing sports or working out at the gym.

Compressive-related back pain and flexion-related back pain are the most likely reason for this type of intolerance, which can be modulated by core stability, glute strength and strength of the posterior lumbar musculature. Sitting and bending impose compressive forces upon the spine, so the primary issue may be flexion-related back pain. 

Pain mostly from minor tasks like opening windows and doors is more likely related to spinal instability, where the core isn’t strong enough to buttress the spine against imposed load.

Back pain from carrying or lifting things in everyday life is generally a result of doing too much, too soon. Tissues such as bone and muscle require time to adapt to the stresses placed upon them.

Poor movement  habits and mobility restrictions may be two other reasons the tissue failure threshold has been reduced.  These movement habits may have also resulted in muscle weakness and incorrect motor patterns, which are the way the nervous system recruits muscles to perform a particular task.

Load intolerance is often related to instability issues, primarily a lack of core stability. However, the stiffening that core stability provides may actually increase pain, which is why accurate assessment is vital.

How we work

A thorough history and assessment will reveal mechanisms of injury, which can include poor movement patterns, dysfunctional motor patterns, muscle weakness, mobility restrictions, perturbed postures and inappropriate loads.

Rehabilitation focuses on first removing pain-causing motions, postures and loads, developing appropriate core stability, teaching better movement patterns and re-establishing correct motor patterns, addressing mobility restrictions and then increasing strength and restoring proprioception, as determined by your goals and needs.
