Extension-related low back pain

Extension-related low back pain is a less common form of low back pain.  It generally only affects participants in specific sports like cricket (bowling), gymnastics, Olympic weightlifting, or throwing sports such as javelin, shot put and hammer throws, or specific occupations, usually tradesman such as electricians, plasterers and mechanics.

It can involve everything from the facet joint, through to conditions such as spondyolosis and spondylolithesis, as well as wedge plate fractures. Often, limited mobility in the thoracic spine may be a reason behind the condition, as well as poor core stability and glute function. Limited thoracic mobility means that the lumbar spine will bend or rotate more than necessary, compromising the passive structures. Poor core stability requires the spine to work harder to stabilise the body.  Similarly, a lack of glute strength means the lumbar spine will do more work, affecting its capacity.

Extension-related back pain doesn’t mean a life in pain. Extension-related back pain doesn’t occur from a once-off motion –  it is the result of the repetitive sub-failure loading in the previous months and years that gradually lowers the tissue failure threshold.

Simply, your previous movement habits are at fault.  These movement habits may have also resulted in muscle weakness and incorrect motor patterns, which are the way the nervous system recruits muscles to perform a particular task.

Dysfunctional movement and motor patterns may be simply habit, or the result of mobility restrictions and muscle weakness from injury.

How we treat extension related back pain

A thorough history and assessment will reveal mechanisms of injury, which can include poor movement patterns, dysfunctional motor patterns, muscle weakness, mobility restrictions, perturbed postures and inappropriate loads.   Rehabilitation focuses on first removing pain-causing motions, postures and loads, building appropriate core stability, teaching better movement patterns and re-establishing correct motor patterns, addressing mobility restrictions and then increasing strength and restoring proprioception, as determined by your goals and needs.

Machine-based training and other systems fail to account for faulty movement patterns, strengthen the body with unnatural movements, fail to allow for nervous system control and ultimately fail when demand exceeds capacity.
